Please open and print the form below. Mail this form accompanied by a

check for your entry fee. All mail entry forms must be received

by September 1st. If it is received after September 1st you will be

notified by phone. After September 1st you will have to register the day

of the tournament by 8:ooam at the Birdseye boat launch in

Stratford, CT. (Map and Directions to boat launch here)
